Group Life Assurance Benefit Nomination Form – Anyone who has membership of a pension should also be completing their pension fund’s own nomination form, which can be found online.
*Please note* It is really important that you have these nomination forms completed for both the Group Life Assurance Benefit and your own Pension to prevent any delays on funds being paid to the beneficiary.
Also below are helpful documents and forms which can be downloaded from MyLothian. All completed forms should be sent to our Payroll team.
